Impact and Programs

Accomplishments 2015 GOALS IMPACT GOAL 1: With 12 community partners, identified the following community mental and chemical health needs - Improve quality processes related to program access and delivery - More licensed chemical health license and youth programs - More early intervention services such as Parent-Child Interaction Therapy - An increased understanding of ADHD GOAL 2: Identified service growth areas for 2016, 2017 and 2018 - Developed 2016 plan to grow Relate's school-based program partnerships in Minnetonka, Wayzata, St. Louis Park, Orono, Mound-Westonka school district and Main Street School of Performing Arts in Hopkins - Accepted an invitation to participate in 3-year chemical health pilot with Univ. of MN - Identified criteria to assess and seek organizations as potential partners in delivery of programs that meet critical client and community needs.
Current Goals 2016 PROGRAM AND SERVICE DELIVERY GOALS: 1- Improve Relate's quality processes by customizing the new electronic records system to better support clinical staff in meeting productivity goals and to reduce staff turnover related to pay. Set and monitor a metric for each staff group and develop and execute individual coaching plans as needed; 2- Reduce wait time for non-emergency appointments to 2 days or less; 3-Initiate curriculum development and pursue license for onsite adult and adolescent chemical health programs with potential program opening in 2017; 4- Train Relate staff in Parent-Child Interaction Therapy, and incorporate P-CIT into community and school based education programs; 5- Engage clinical staff in producing original mental health articles with the acceptance of one Relate article per month by a local publications; 6- In partnership with Relate's school based programs in Minnetonka, Wayzata, St. Louis Park, Orono, Mound-Westonka districts and Main Street School of Performing Arts in Hopkins assess the current service level to service needs, set performance metrics and adjust or hire staff to meet agreed to outcomes; 6- Complete paperwork for Univ. of MN 3-year chemical health pilot and initiate program; 7- Obtain funding to pilot placement of a mental health provider in 1 to 2 local food shelves, identify and measure agreed to metrics; 8- Using the partner selection criteria, identify 2 or more potential collaboration partnerships that focus on adults and seniors
Community or Constituency Served Individuals of all age ranges, with a focus on children, adolescents and adults living in the western Minneapolis suburbs. School based counseling to students, families and staff; consultative services to Head Start programs; employee assistance to employers; and education presentations to the community.
Geographic Area Served We serve the western suburbs of Minneapolis, Minnesota, including Hennepin and Carver Counties.
